The Trustees and Staff of the Georgia Guard Insurance Trust are happy to announce that you and your family can now benefit from one of three dental plans offered by Fortis Dental Care, an A+ rated insurance company. Fortis is recognized as a national leader and innovator in the area of voluntary dental plans. Dental coverage is second to health benefits as the most popular benefit available today. Now is the time to secure this coverage for yourself and your family. ALL APPLICATIONS AND INITIAL QUARTERLY PREMIUMS MUST BE RECEIVED BEFORE THE 5th OF THE MONTH to have an effective date the first of the next month.

Payment options include credit union draft from your Georgia Guard Credit Union account, bank draft, credit card, allotment if your employer allows it, or monthly, quarterly, semi-annual, or annual payments as you desire. See application for details.

1. Network Based Plan - A dental HMO (DENTICARE) - Available only in areas where providers are contracted. You get great value with the lowest premium here. Participating Dentists offer significantly reduced fees.
   
2. Basic Option Coverage - FREEDOM BASIC - Covers preventive and minor restorative work only. No Major services covered. Use any licensed dentist. Claim forms are filed. This plan has a $50 per person deductable and a $1,000 per person maximum per benefit period.
   
3. Deluxe Option Coverage - FREEDOM ADVANCE - Covers preventive, minor and major dental care. Covered major care is reimbursed at increasing levels over time. This coverage requires the highest premium. This plan has a $50 per person deductible and a $1,000 per person maximum per benefit period.

You also receive a discount vision plan at no additional cost regardless of the plan you choose. You can save on eye exams, lenses and frames too!
