FREEDOM BASIC
Basic Option Coverage - Covers preventive and minor restorative work only. No Major services covered. Use any licensed dentist. Claim forms are filed. This plan has a $50 per person deductable and a $1,000 per person maximum per benefit period.
FREEDOM ADVANCE
Deluxe Option Coverage - Covers preventive, minor and major dental care. Covered major care is reimbursed at increasing levels over time. This coverage requires the highest premium. This plan has a $50 per person deductible and a $1,000 per person maximum per benefit period.

ALL APPLICATIONS AND INITIAL QUARTERLY PREMIUMS MUST BE RECEIVED BEFORE THE 5th OF THE MONTH to have an effective date the first of the next month.
Payment options include credit union draft from your Georgia Guard Credit Union account, bank draft, credit card, allotment if your employer allows it, or monthly, quarterly, semi-annual, or annual payments as you desire. See application for details.
